On May 20th, 2026 the East Harlem Council for Human Services, Inc. hosted its 61st Anniversary Gala at CAPITALE 130 Bowery, NYC. The theme of our celebration was “Saving Lives, Building Futures Con Corazón y Comunidad. Our organization’s rich history and our life-changing work was on full display during this unforgettable evening. The organizations and special individuals we recognized this evening have been integral in helping us improve the lives of individulas in our communites and share our mission wholeheartedly. Throughout the evening, we showcased our plans for the future and we hope that everyone in attendance and who support our organization will help us as we take these next steps. We are so grateful for those who were able to spend the evening with us and we hope to see you again in the future. The East Harlem Council for Human Services, Inc. (Council) was established in 1965 through the efforts of East Harlem residents and their struggle to provide crucially needed services to their community. The board and staff are predominantly community residents and users of services who have a strong commitment to providing the highest quality of fully bilingual health and human services to the East Harlem community.

The East Harlem Council for Human Services, Inc. operates the following fully bilingual Programs, serving more than 12,000 individuals each year without regard for an individual’s ability to pay for services: Boriken Neighborhood Health Center, Boriken School Health Program, Bilingual Head Start Program, and the Senior Nutrition Program.

President and Chief Executive Officer of Urban Health Plan (UHP)

Paloma Izquierdo-Hernandez is the President and Chief Executive Officer of Urban Health Plan (UHP), one of the largest community health center networks in New York State and a nationally recognized leader in promoting health equity. She is a visionary and a respected thought leader who has dedicated her career to enhancing access to high-quality, affordable healthcare while addressing the social drivers of health that impact the communities served by UHP.

Under Paloma’s leadership, UHP has expanded from a single health center in the South Bronx to a network of 13 federally qualified health centers located throughout the Bronx, Central Harlem, and Corona, Queens. Additionally, UHP operates 12 school-based health centers, two mental health facilities, three WIC sites and a variety of many other grant-funded programs. Today, UHP provides health care services to over 90,000 patients each year.

Paloma believes that meeting basic needs is essential for individuals to achieve optimal health. She co-founded the Hunts Point Longwood Community Coalition, which, at a very micro neighborhood level, comprises key community stakeholders who help integrate social and human service programs to address food insecurity, housing issues, and job preparedness. Additionally, she has spearheaded significant community development projects and partnerships, understanding that economic growth and sustainability are crucial for reducing health disparities. In 2015, she enhanced UHP’s role in local development by incorporating the Greater Hunts Point Economic Development Corporation as a wholly owned subsidiary of the organization.

With great pride, Paloma continues the work and vision of her late father, Dr. Richard Izquierdo, UHP’s founder. In 2010, she, together with Dr. Izquierdo and Urban Health Plan, established the Dr. Richard Izquierdo Health & Science Charter School to address educational gaps and create a pipeline for future health professionals. The school has graduated over 700 students, has consistently achieved high college acceptance rates, and is building opportunities for the next generation, including serving on the school’s own board of trustees.

Under her leadership, a culture of patient-centered care, teamwork, and continuous improvement has thrived at UHP. As a result, the organization has received national recognition, including designations as a high-performing health center, exceptional performance in clinical measures, and numerous awards for innovation and quality. In 2025, UHP celebrated new and expanded partnerships under her leadership, including the launch of an ACGME-accredited Community Psychiatry Residency Program to address the growing need for mental health services. This year, Paloma also oversaw the completion of UHP’s newest health center in the Bronx, Plaza San Juan. This state-of-the-art, 52,000-square-foot healthcare facility expands access to quality primary care by significantly increasing the amount of subspecialty medical care provided to its patients. The four-story health center will also contribute to economic development and environmental renewal in the area.

Paloma, who was raised in the Bronx, is a graduate of Boston College. She holds a Master of Science in Speech Pathology from Columbia University’s Teachers College and a Master of Public Health from Columbia University’s Mailman School of Public Health.

She has also been awarded honorary doctoraldegrees from Boston College, the Metropolitan College of New York, and Mercy University.Paloma serves on multiple local and national boards and currently chairs the National Association of Community Health Centers (NACHC). In 2024, she was inducted into the Bronx WalkofFame and received the People’s Choice Award in Healthcare. She remains deeply committedtoadvancing Urban Health Plan’s mission and ensuring healthier, more equitable futures forallcommunities.

Steven Luciano Rivera, a proud New York Bronx–Puerto Rico native, has dedicated the past 28 years to transforming healthcare and uplifting underserved communities. His career began at NYC Health and Hospitals Corporation, later serving as VP of Marketing and Business Development for top plans like Healthfirst and HealthPlus.

A visionary entrepreneur, Steven co-founded Sentry Management Solutions in 2016, revolutionizing non-emergency medical transportation. In 2021, Sentry became the first NY State-certified minority-owned business to win the largest contract in MTA history.

In 2022, Steven launched ProHealth Connect, a national network of bodegas, pharmacies, and supermarkets — providing OTC and grocery benefits to underserved communities. By June 2025, the platform reached 1 million members across 50 states and U.S. territories, driving over half a billion dollars in sales.

In 2024, Steven was appointed President of the New York State Federation of Taxi Drivers, representing 30,000 NYC drivers. He now leads advocacy efforts focused on industry standards, resources, and working conditions.

Though he once dreamed of politics, Steven found purpose in business leadership — building innovative solutions to improve lives across New York and beyond.

NYC Health + Hospitals

NYC Health + Hospitals, the nation’s largest public health care system, was founded in 1969 as the New York City Health and Hospitals Corporation (HHC) to unify and stabilize the City’s public hospitals and clinics at a time of financial crisis and fragmented governance. Built on a legacy that dates to the 18th century, the health system includes historic institutions including Bellevue, the country’s oldest public hospital. The mission was to guarantee access to care for all New Yorkers, regardless of ability to pay or immigration status. 

Over decades, it evolved into a comprehensive, citywide network of hospitals, community-based clinics known as Gotham Health, long-term care facilities, and correctional health services. In 2016, HHC rebranded as NYC Health + Hospitals to communicate its mission, identity, and connection to the communities it serves. That unified identity proved essential during moments of crisis—most notably the COVID-19 pandemic—when the system stood as the backbone of New York City’s public health response.

Hispanic Federation

Hispanic Federation (HF) is a nonprofit membership and advocacy organization, founded in 1990, committed to empowering and advancing the Hispanic community, with a focus on low-income, marginalized, and immigrant Latine.

Hispanic Federation serves as a national model for Latino social, political, and economic empowerment.  With programs in 43 states and territories and a strong presence in New York, Florida, North Carolina, Puerto Rico, and other key states throughout the U.S., HF works to uplift millions of Hispanic children, youth, and families across the country.  In addition, our Washington, DC office gives HF a true national advocacy presence.

HF works locally, state-wide, and nationally to strengthen Latino nonprofits, promote public policy advocacy, and bring to scale a portfolio of innovative community programs.  HF’s focus areas include immigration, economic empowerment, civic engagement, disaster relief, philanthropy, education, health, and the environment. HF also maintains ongoing public education campaigns and meets the organizational development needs of its member agencies through grant-making and capacity-building assistance.
